Click on the link below to submit your application online:
1.) Click on the link below (LivingLinks Application and Availability Form), then save the “Application & Availability Sheet PDF” to your computer as a document (right-mouse click on the link and choose “Save file as” for Internet Explorer and “Save link as” for FireFox). To complete and email the file back to us, open the document and click on the “T typewriter” at the top of the heading and enter information. Save as a document, then email back as an attachment to firstname.lastname@example.org.
2.) Download a PDF of the application and availability form using the link below – then print, complete, and mail or hand deliver it to the Central Office at 1230 North River Drive. You may also fax it to 507-345-3771.
3.) Visit our Central Office located at 1230 River Drive in Mankato and fill out the application and availability sheet on-site.
If you are called for an interview, you will be required to authorize LivingLinks in writing, permission to access a copy of your driving record and conduct a Department of Human Services (DHS) background check which includes fingerprinting.
LivingLinks’ insurance company will not allow a disqualified driver to transport people we serve because transportation is an essential job function for nearly all positions at LivingLinks. Examples of disqualification include (in the past 5 years):
- Three or more speeding tickets, and/or tickets for speeds above 10 mph over the posted speed limit.
- Driving without a license/driving after revocation.
- Vehicular bodily injury/vehicular homicide (also a disqualification by the Department of Human Services).
