Individual Program Plans

Individual program plans are an integral part of our clients' lives. A program plan is written specific to each client and is designed to teach or maintain a particular set of skills. Qualified staff members write, implement, and document each program plan. The goal of promoting as much independence as possible, and the best interest of the client are foremost when each plan is written and carried out.

Intermediate Care Facilities (ICF/MR)

Two intermediate care facilities are located in a quiet residential area of upper North Mankato. Each facility allows the eight people living there to express their individuality and increase independence in a comfortable, homelike setting. The facilities each have eight bedrooms, a kitchen and dining area, living room and a recreation room. Our dedicated staff provide 24-hour, supervised care. They also provide support for the individuals in the areas of self-care, daily living skills, recreational activities and community integration.

Supported Living Services (SLS)

LivingLinks has four of these waivered sites that provide residents with all the comforts of home. One site is a house with a nice yard near MSU-Mankato. Another SLS site is a cozy house in the middle of a quaint neighborhood in lower North Mankato. We also have a home near the Belle Mar Mall. Independence is encouraged and life enriched by a variety of activities. Adaptive equipment is available to make life more enjoyable. The qualified staff schedule and accompany all appointments the client may need.

In-Home Family Support Services

This service provides staff support to individuals and their family members. One type of support provided by family support specialists is to teach and assist with life skills, such as communications, self-care and skill training. Support is also offered in the areas of activities and community integration. The family support specialists and the individuals also participate in activities "in home" and in the community.
